MEMORANDUM OF UNDERSTANDING (MoU)

Khidmat-e-Khalq Foundation,
Registered Office: 301, 3rd Floor, Qadri Home, Near VK English School, Govandi, Mumbai – 400043
(Hereinafter referred to as “Foundation”)
And
Impulse Multispeciality Hospital,
Represented by Dr. Irfan Sayyed, Director,
Located at Shahnai Hall ke paas, Beganwadi, Govandi, Mumbai.
(Hereinafter referred to as “Hospital”)
Purpose:
The purpose of this MoU is to establish collaboration between Khidmat-e-Khalq Foundation and Impulse Hospital to provide affordable and quality healthcare services to underprivileged and needy patients.
Terms of Understanding:
1. Affordable Services:
The hospital agrees to provide the following discounted services to patients referred by Foundation:
General Physician consultation and OPD medicines @ ₹50
Blood, Urine, Sugar tests @ up to 50% discount
Sonography and X-ray @ up to 30% discount
Pharmacy medicines @ up to 15% discount
2. Specialist Consultations:
Hospital will ensure availability of on-call specialist doctors including General Physician, Cardiologist, Gynecologist, Pediatrician, Orthopedic, Neurologist, Nephrologist, Gastroenterologist, Chest Physician, ENT Surgeon, and Psychiatrist at standard discounted rates for foundation patients.
3. Surgery Facilities:
Hospital will provide surgical procedures such as Normal Delivery, C-Section, Gallbladder, Kidney Stone, Hernia, Piles/Fissure/Fistula, Appendix, Hydrocele/Varicocele, Hysterectomy at affordable charges as mutually discussed.
4. Foundation Role:
Foundation will identify and refer deserving patients and create awareness regarding this facility among its beneficiaries.
5. Duration & Termination:
This MoU will remain valid for one year from the date of signing and can be renewed with mutual consent. Either party may terminate this MoU by giving 30 days written notice.
6. General Clause:
Both parties agree to work with transparency, honesty, and dedication towards the welfare of society under this MoU.
